Annual Physical Exams

Comprehensive health evaluations including vital signs, physical examination, and health history review. For women, exams may include breast and pelvic screenings. For men, testicular and prostate exams when appropriate. We also provide preventive screenings based on your age and risk factors.

Mental Health

Chronic Condition Management

Ongoing care for diabetes, high blood pressure (hypertension), high cholesterol, asthma, COPD, heart disease, thyroid disorders, and other long-term health conditions. Regular monitoring, medication adjustments, and lifestyle support to keep your condition stable and prevent complications.

Preventive Care & Screenings

Immunizations, cancer screenings, cholesterol testing, diabetes screening, blood pressure monitoring, and other preventive services designed to catch health issues early. Personalized screening schedules based on your age, family history, and health risks.

Acute Illness & Sick Visits

Treatment for flu, colds, respiratory infections, urinary tract infections, allergies, skin conditions, minor injuries, and other non-emergency health concerns. We diagnose and treat acute illnesses to get you feeling better quickly.

Weight Management & Nutrition Support

Medical evaluation and support for weight-related concerns. Nutritional guidance, lifestyle counseling, and treatment for conditions affected by weight such as diabetes and high blood pressure. Personalized approach based on your health goals and medical needs.

Medication-Assisted Treatment

Medication Management & Coordination

Review and management of all your medications to ensure they work together safely. Prescription refills, medication adjustments, and coordination with specialists when needed. We monitor for drug interactions and side effects, especially when you're seeing multiple providers.

Your doctors should talk to each other

When your primary care doctor doesn't know what your psychiatrist prescribed, or doesn't understand how substance use affects your diabetes — important things get missed. You end up coordinating everything yourself.

When your providers work together

Medications stay safe 
Some psychiatric meds affect blood sugar or cholesterol. Substance use impacts liver and heart health. Your primary care doctor monitors these and adjusts treatment.

Fatigue, sleep problems, weight changes : sometimes these are related to depression, anxiety, or other behavioral health concerns. Connected care catches these patterns.

Physical health impacts mental health and vice versa. When your providers coordinate, you get care that actually fits your life.

First Visit

What to bring: 

• Insurance card & ID 

• List of current medications (including over-the-counter and supplements) 

• Medical records if you have them 

What happens (45-60 min): Your provider reviews your health history, does a physical exam, and may order lab work. We ask about mental health, substance use, sleep, and stress because they impact your physical health. 

You’ll leave with a care plan and a follow-up appointment scheduled.

We work directly with most health insurance plans in Massachusetts for all services. Some of the plans we accept are Masshealth, Medicare, Cigna, Aetna, Blue Cross Blue Shield, Harvard Pilgrim, Wellpoint, WellSence, Mass General Brigham, and more.
